Why Is It So Highly Demanded?

The main reason this aesthetic approach has become so popular is that people are now avoiding an obviously “done” aesthetic and are seeking unnoticed beauty. Russian-type nose aesthetics is one of the most elegant methods that meets this expectation. When examined technically, this approach is a procedure in which the dorsum of the nose is gently corrected, but the real magic happens at the nasal tip.

In traditional rhinoplasty, the nasal tip could sometimes be left overly sharp or pointed. In this technique, however, the nasal tip is lifted by creating a slight upward rotation while being left rounder and softer in form. This “ball-shaped” tip structure adds a younger, fresher, and more innocent expression to the face, especially for women. It removes harshness from the face and softens the gaze. Most of our patients want to see the best version of their own face in the mirror rather than an aggressive change. This technique aims to preserve and enhance the existing anatomy by causing minimal damage to the bone and cartilage structures.

Who Is This Aesthetic Suitable For and Does Ethnic Origin Matter?

Every face has a different story, and not every surgical technique is applied to every patient. Russian nose aesthetics yields the best results especially in individuals of the “Caucasoid” race, with not very thick nasal skin and cartilage structures with a certain level of strength. In this patient group, there is usually a slight hump on the nasal bridge or a mild drooping of the nasal tip. The procedure required is not to recreate the nose from scratch, but to refine the existing structure by filing it down and lifting the tip.

This should not be confused with “ethnic rhinoplasty” applied to Asian or African patients. For example, an Asian patient usually has a very low nasal root and thick skin; in such cases, the nose needs to be elevated and augmented. In contrast, for patients who desire the Russian technique, we generally gently reduce and shape the nose.

The characteristics of the most suitable candidates for this aesthetic approach are as follows:

Slightly humped nasal structure

Drooping nasal tip

Thin or medium-thickness skin

Balanced facial features

Strong cartilage support

How Are Ideal Nasal Tip Rotation and a Natural Appearance Achieved?

In aesthetic surgery, working “by eye” is not possible; every millimeter has its own mathematics. When patients say, “I want my nose to be upturned but not look like a pig nose,” they are actually referring to a scientific concept. To achieve the characteristic stylish posture of Russian nose aesthetics, we must very precisely adjust what we call the “Nasolabial Angle,” which is the angle between the upper lip and the base of the nose.

In women, an angle between 95 and 100 degrees is ideal. This range provides the desired feminine and attractive rotation. However, when it exceeds 100 degrees, the inside of the nostrils begins to be visible from the front, which is one of the least desired appearances in aesthetic surgery. In male patients, this angle is kept lower to preserve a masculine expression.

Additionally, what we call “Projection,” meaning how far the nose protrudes from the face, is also very important. To prevent the nasal tip from being excessively sharp or overly flat, we calculate its proportion with other facial components (Goode Ratio). All this planning is done so that when the patient leaves the operating table, the result does not shout “I am here,” but instead quietly harmonizes with the face.

Are Breathing Problems Corrected During the Aesthetic Process?

No matter how beautiful a nose looks, if the patient cannot breathe comfortably, it has no importance. In every patient who applies to our clinic with aesthetic concerns, the first thing we examine is the airway. Many times, patients become accustomed to internal problems because they focus on external appearance. However, for us physicians, function always comes before form, or at least is equally important.

Before surgery, we measure the patient’s intranasal airway resistance using devices such as “Rhinomanometry.” If there is a bone deviation (septal deviation) or turbinate enlargement (concha hypertrophy), aesthetic surgery is a great opportunity to solve these problems. With the combined surgery we call “Septorhinoplasty,” we both correct the external hump and open the internal airway.

The main causes that lead to breathing problems are as follows:

Septal deviation

Turbinate hypertrophy

Nasal valve narrowing

Presence of polyps

Chronic sinusitis

Is Non-Surgical Russian Nose Filler Possible?

Not everyone may be ready or willing to undergo surgery. At this point, “non-surgical rhinoplasty,” or popularly known as nose filler, comes into play. This method can be a miraculous solution especially for patients who do not want to go under the knife but are seeking small touch-ups on their nose. By using fillers containing hyaluronic acid, it is possible to shape the nose in a short time of 15–20 minutes.

With a small touch to the nasal tip using fillers, we can temporarily mimic that “upturned” Russian nose effect. We can also make the profile perfectly straight by filling in front of and behind slight humps on the nasal bridge. The most beautiful aspect of this procedure is that the result is seen immediately and the patient can return to social life right away. Moreover, since fillers can be dissolved with the enzyme “Hyaluronidase,” it is like a reversible, low-risk “test drive” if the patient does not like the result. However, it should be remembered that fillers do not reduce the size of the nose; they only shape it and add volume.

What Are the Safety Risks in Filler Procedures?

Although nose fillers are sometimes portrayed on social media as a very simple cosmetic procedure done during a lunch break, they are actually medical procedures that require serious anatomical knowledge. The nose is one of the most complex regions of the face in terms of blood vessels. Injections performed by unskilled hands can lead to vascular occlusions and tissue loss. Therefore, safety measures such as the use of blunt cannulas or needle-free techniques are of vital importance.

The person performing this procedure must be a physician who has mastery of facial anatomy. When performed by an expert, risks are minimal and results are highly satisfactory. However, patients should adjust their expectations knowing that this procedure is temporary (on average 12–18 months) and does not provide a permanent structural change.

Situations in which filler application is not suitable are as follows:

Large nasal structure

Severe deviation

Breathing problems

Active skin infection

Pregnancy period

How Is Permanence Achieved in the Surgical Method?

Fillers are temporary, but surgery (rhinoplasty) is lifelong. In Russian nose aesthetics, the most critical success of the surgery is ensuring that the upturned and elegant nasal tip we aim for does not succumb to gravity and droop over time. In older techniques, shape was sometimes given only with sutures, which could lead to tissue relaxation over time.

Today, we structurally support the nose as if constructing a building. We place cartilage taken from inside the patient’s own nose (septum) into the nasal tip like a column or pillar. These cartilage supports, which we call “strut grafts,” ensure that the nasal tip remains at the ideal angle even years later. Thanks to this, even 10 years after surgery, your nose maintains its original form and rotation. Surgery is not just about shaping, but about engraving that shape into the tissue’s memory.

What Awaits Patients During the Recovery Process?

The most frequently asked question by patients who decide to undergo surgery is the recovery process. Surgical rhinoplasty is a journey of patience. In the first week, there is a protective splint and tapes on the nose. When this splint is removed, the patient sees their new nose for the first time; this is a very exciting moment, but the nose seen is not yet the final result because swelling is present.

Since we use tissue-respecting, minimally traumatic methods in the “Russian Nose” technique, bruising and swelling are much less compared to the past and resolve more quickly. However, full settling of the nose and the skin wrapping around the new framework can take between 6 months and 1 year. During this period, there are some golden rules that our patients must follow:

Things to pay attention to during the recovery period are as follows:

No wearing glasses

Salt restriction

Protection from the sun

Pillow height

Protection from trauma

How Does Nose Aesthetics Affect Other Areas of the Face?

Facial aesthetics is a whole, and the nose is just the central piece of this puzzle. Sometimes our patients come thinking their nose is very large, but in our analysis we realize that their chin is actually retruded. In a patient with a recessed chin, the nose appears larger and more prominent than it actually is.

To achieve the baby-like and balanced atmosphere of “Russian Nose” aesthetics, sometimes working on the nose alone is not enough. To achieve profile balance (Nasomental Angle), small touches to the chin (Mentoplasty) may be required in the same session as rhinoplasty. A chin implant or fat injection placed on the chin tip greatly supports the new shape of the nose and creates a flawless profile. That is why we surgeons focus not only on the nose but on the entire profile from the forehead to the chin.

How Is the Decision Made Between Surgery and Filler?

So, which path should the patient choose? This decision is entirely based on personal needs and anatomical realities. If the patient wants immediate results, does not have time for a recovery period, needs only millimetric corrections on the nose, and permanence is not a priority; fillers are a great option. They are also a good starting point for those who have a fear of surgery.

However, if the patient has breathing problems, a prominent nasal hump, wants to reduce the size of the nose, or says, “I want to do it once and have it stay with me for life”; surgery is the only and most correct path. As surgeons, our duty is to guide the patient toward the most appropriate treatment. Sometimes patients want fillers, but their anatomy is not suitable; in such cases, we honestly recommend surgery. Sometimes we see that a patient who wants surgery could actually be satisfied with a small filler.
